Zulfiqar Bhutta: patriotics and paediatrics

Nobody is really sure what Samuel Johnson had on his mind when he said that “patriotism is the last refuge of the scoundrel”, but it's a safe bet that he wasn't referring to the kind of patriotism practised by Zulfiqar Bhutta. From his base at the Aga Khan University in Karachi, Bhutta has gone about the task of improving child and maternal health in his native and beloved Pakistan with missionary zeal, while at the
same time putting child survival and maternal and child undernutrition front and centre on the world stage.

Bhutta's patriotism is a complex blend of duty, loyalty, pride, and concern for a country that faces existential challenges. “Pakistan has many dimensions of poverty”, says Bhutta in his rich, resonant baritone, each word delivered with a gentle precision. “There is the poverty that you and I can see out on the streets, a poverty of means”, he continues, “but there is also an enormous poverty of hope and poverty of imagination, even among the learned classes. People
think there are just no innovative solutions.”

Needless to say, Bhutta is not one of those people. “In my heart of hearts I feel very optimistic”, he says. “Even amongst the conflict and the societal fissures, I see the seeds of social change.” There is certainly no doubting Pakistan's resilience. In the devastating earthquake of 2005 and the floods of 2010, Pakistan went through two humanitarian catastrophes and yet emerged unbroken with minimal assistance from the outside world. Bhutta had first-hand experience of organising the response to both disasters, and took heart from the compassion and collectivism he witnessed in their aftermaths. “I haven't given up on the hope that from the ashes we see around us will rise a phoenix one day, and there will be good governance, and there will be leadership, and Pakistan will emerge stronger than before”, he says. Whatever the Pakistan of tomorrow looks like, Bhutta, together with
his wife and academic collaborator Shereen, will have played a major role in shaping it for the better. From an educated and
forward-looking family, a young Bhutta had his eyes opened to the “grinding poverty, undernutrition and inequities of a nature I'd never seen before” during a study tour of what was then East Pakistan (now Bangladesh) in 1969, the year before he went to university. 7 years later he graduated from the Khyber Medical College at the University of Peshawar with the title of “graduate of the year” to his name, and embarked on a journey that has transformed maternal and child health
in Pakistan and internationally.

After overcoming the withering condescension that met his first efforts to train in paediatrics in the UK, Bhutta returned home in the mid-1980s at a time when neonatal paediatrics was still virtually unrecognised in Pakistan. Many would have balked at the scale of the challenge, but Bhutta saw it as an opportunity to build something from the ground up. “We [the first-person singular rarely crops up when Bhutta talks about his work] capitalised on the fact that we started out around the time when people were really clamouring for some work in this area, and we were able to walk the talk. So not only did we do
the advocacy and talk about what needed to be done, but we were actually able to demonstrate in country that it could be done.”

“He has these unique opportunistic and strategic abilities with which he has influenced change where it matters, even in limited resource settings”, says Zohra Lassi, a colleague of Bhutta's at the Aga Khan University and one of hundreds to have trained under him. At the same time as doing randomised trials on a range of different interventions—from community-based interventions to the application of chlorhexidine to neonatal umbilical cords—Bhutta was able to build the evidence base that has underpinned improvements in child and maternal health while simultaneously building the capacity to deliver them. The Division of Women and Child Health at Aga Khan University, which Bhutta helped found from scratch, now has more than 1000 members, making it the largest in Asia.

“His commitment is an excellent demonstration of what can be achieved in building research capacity within a developing county”, says Alvin Zipursky, Chair and Scientific Director of the Programme for Global Paediatric Research, who notes that Bhutta's international importance is evidenced by his appointment to the independent Expert Review Group
(iERG) that was created earlier this year by the UN Secretary General to follow up the work of the Global Strategy for Women's and Children's Health and the Commission on Information and Accountability. And from his “slightly dissenting” Comment on new figures published in The Lancet for global, regional, and national causes of child mortality, it's clear that Bhutta will be pushing to find ways of “going beyond the mortality numbers, to try to understand some of the social determinants and the dimensions of what the numbers mean. Without addressing those fundamental social determinants, trying to achieve change through technical interventions is going to be very difficult.”

“I'm a global citizen in many ways”, says Bhutta of his role on the iERG, “and I recognise the primacy of my responsibility to my own country, but also now increasingly the responsibility to the developing world at large. So while I have a very nationalist attitude I also see that the globe is shrinking, and that many of the things that we've achieved in Pakistan need to be emulated in other parts of he world.” If Johnson was right about patriotism, the world needs more scoundrels like Bhutta.

Violence against people with disabilities is an important public health and human rights concern. There is increasing attention to violence against people with disabilities in the United Kingdom and other countries around the world, with increased media reporting on the topic and growing public awareness of hate crime. Additional research is required to better understand the issue.
 
In line with a public health approach to violence prevention, WHO has commissioned Liverpool John Moores University to produce systematic reviews of data on the prevalence and risk of violence against adults and children with disabilities. Published in The Lancet, the first of these reviews has indicated that adults with disabilities are at much greater risk of violence than adults without disabilities. In fact disabled adults are 1.5 times more likely to be a victim of violence than those without a disability, while those with mental health conditions are at nearly four times the risk of experiencing violence.
 
 
The systematic review of violence against children will be published later this year. To inform and improve policy and practice in the field, further research is required to explore risk and protective factors and underlying causes for violence, as well as evidence for interventions which either help prevent violence against people with disabilities in the first place or improve outcomes for victims.  
 
In the context of the Disability Studies Association annual conference in Lancaster, United Kingdom, to be held from 11-13 September 2012, WHO will host a roundtable presenting the evidence from the systematic reviews, together with papers on risk and protective factors and on the evidence for the effectiveness of interventions. In this regard, WHO is issuing a call for papers based on empirical data, whether from quantitative or qualitative social research. Papers which discuss the situation in low-income or middle-income countries will be prioritized. If you are a researcher, policy-maker or practitioner working in this area, and you would like to present a paper on violence or interventions to prevent violence, please contact Tom Shakespeare (shakespearet@...). Two bursaries of Ł500 are available to support participation by researchers from low-income or middle-income countries. The deadline for expressions of interest and abstracts is 16 April 2012.

 THE YOUNG NEUROSURGEONS SUPPORT


 

Turkish Neurosurgical Society will provide sponsorship for 45 young neurosurgeons (younger than 40 yrs) from developing countries for ACNS 2012 Congress. The list of the developing countries is indicated by International Monetary Found. There will be a quote for each country in respect to population.

The sponsorship will consist of free accommodation, registration, food and social activity fees during the ACNS 2012 Congress.

The applications will be evaluated by a Grant Board.

 

The documents needed for application are;

1-   CV

2-   Abstract of oral presentation

3-   Two reference letters (one from the chairperson of the institution, one from the president of the national neurosurgical society)  

 

The deadline for application is May 15th, 2012

COURSE 1 : Minimally invasive approaches to lumbar spinal surgery

NUMBER OF ATTENDEES : 30

The aim of the course is to provide advanced information about minimally invasive spinal approaches. This course is designed to create an environment in which students understand and challenge the use, advantages and limitations of the minimal invasive techniques.  Applications for participation can be submitted through the registration page of the course. A total of 30 students will be selected from pre-inscriptions received. The official language of the meeting is English. A certificate of attendance will be presented to all registered participants.

This course will be held at the American Hospital, in Istanbul, on Sept 02, 2012. For detailed information please contact;

 

COURSE 2 : A Hands-on training neuroendoscopy workshop

NUMBER OF ATTENDEES : 50

Program Includes:

•     Hands-on workshop on endoscopic models

•     Handling of the endoscopic material

•     Interactive discussion of selected videos

Lectures on:

•     Neuroendoscopic anatomy of the ventricular system and periventricular structures

•     Basic endoscopic equipment and surgical technique

•     Current neuroradiological investigation techniques in hydrocephalus

•     Pitfalls and complications of cranial neuroendoscopy

•     Neuroendoscopic treatment of hydrocephalus and patient selection criteria

•     New insights on hydrocephalus and future perspectives

•     Case presentations, decision  making in selected cases with interactive discussion

 

COURSE 3 : The Impact of White Matter Anatomy for Microneurosurgical Procedures

NUMBER OF ATTENDEES : 80

Program Includes:

•     Cadaver Brain dissection instructing fiber system of the human brain. 

Lectures on:

•     Advanced Tehcniques in Fiber Tractography,

•     Microneurosurgery of primary brain tumors in neopalleal, limbic system, intraventricular and brain stem.

 

This course will provide an overview of the anatomy of white matter tracts via demonstration of white matter dissection in cadaver brain to residents in neurosurgery and neurosurgeons interested in brain surgery in every level. Advanced techniques in fiber tractography and its usage in neurosurgical practice will be presented. With case discussions and surgical videos the faculty will provide the audience with "pearls" from their surgical experience for excision of intrinsic brain tumors.

 

COURSE 4 : PNLS Course

NUMBER OF ATTENDEES : 50

The course developed in Japan for neurosurgical medical staff (Dr, Ns, and also EMT) to learn and practice assessment and management of neurosurgical emergency diseases.  The content of the PNLS course as follows.

1) Coma scale

2) Neurological evaluation

3) Systemic management including cerebral herniation

4) Group work

We mainly focus on novel ideas, discoveries, hypothesis, minimally invasive surgical techniques, and innovative drugs/instruments. Our journal is currently indexed in Index Copernicus, plan to index in Pubmed Central soon. We are introducing distinguished online manuscript processing system which allows submission of articles with tracking of its progress till proof stage.
The average review time for MEHDI in ophthalmology is 29 days from the point of submission until a final decision has been reached.
