1)     Improving your performance during an emergency OSCE station:
":
Rapid Primary Survey & resuscitation:
 ABCDE
 
A Airway maintenance with C-spine control
B  Breathing and ventilation.
C Circulation (pulses, hemorrhage control).
D  Disability (neurological status).
EExposure (complete) and Environment (Temperature control).
Restart ABCDE if patient deteriorates.
Always deal with A & B first as they may kill the patient now not C.
Airway:
1. Â  Immobilize cervical spine with collar or sand bags. In Trauma case ONLY.
 
To the examiner/ nurse "A Collar or sand bags please to immobilize the C-spine."
 
2. Â  Airway assessment: Assess ability to breath and speak.
If patient is already responded appropriately to you so far, indicates patent airway & ability to breath is normal.
"Mr./Ms.., Where are you now?…What day of the week is today?"
 
To the examiner  "Patient is alert, oriented, speaking, no noisy breathing, airway is patent."
 
3. Â  Airway management:
 
 
GO TO BREATHING if airway is patent.
OR  To the examiner "Patient is…., an indication for endotracheal tube."
2- Definitive airway management:
-  Endotracheal intubation (ETT):
Find out them at the ebook " A Step By Step Guide To Mastering The OSCEs"
Breathing:
1- LOOK: for:
  1. Mental status: anxiety, agitation.
  2. Color: cyanosis / pallor.
 3. Chest movements.
 4. Respiratory rate & effort.
  5. JVP. (if collar on don't remove it. If sand bags, do it.)
To the examiner  "Patient is not agitated, no cyanosis or pallor. Normal symmetrical chest movements, Normal respiratory effort & rate at…bpm, JVP is...."
2- FEEL: for: 
  1. Airflow.
2. Tracheal shift. (if collar on don't remove it. If sand bags, do it.)
  3. Chest wall for crepitus.
 4. Flail segments & sucking chest wounds.
5. Subcutaneous emphysema.
 "Mr/Ms..., I'm going to uncover and feel your neck and chest, Okay."
 "Any pain?"
To the examiner "There is no tracheal shift, crepitus, flail segments, sucking wounds or subcutaneous emphysema."
3- LISTEN:  Â Ã‚ Ã‚ Ã‚  1. Sounds of obstruction (Stridor) & Air escaping.
2. Breath sound & symmetry of air entry. Both sides: apex, lower, & sides.
3. Heart sounds. If muffled with high JVP: Temponade: pericardiocenthesis.
  "Mr/Ms..., I'm going to listen your chest."
To the examiner "Breath sounds are normal, symmetrical, no stridor, normal heart sounds / …. Â Ã‚  diminished air entry on the left….."
4- Assess Respiratory Function:
 
 Â Ã‚  Ventilation modalities:
 
Circulation:
1.  Ask for Vital signs:
 
To the examiner/nurse "What are his/her vitals, please?"
Carefully listen to what the examiner says and comment: e.g. "Normal/ so, he has fever/tachycardia/ tachypnea….".
 
- Blood pressure: If conscious mobile patient, take it on:
"Mr/Ms.., I'm going to check your blood pressure in both your arms then your leg, (if no collar) and I'll recheck your arm while sitting/standing for a minute?."
Note: Usually the examiner will stop you and give you the results, but start doing it until he/she stops you.
Measure BP in unconscious yourself on one arm.
 
To the examiner "Blood pressure is …. mmHg lying and …. mmHg standing. No postural hypotension, No significant upper/lower extremities difference. (aortic dissection)"
 
To the nurse "Put him on cardiac monitor & pulse oxymeter. Repeat vitals every (5-15) minutes & inform me."
 
2. IV lines & Investigations:
 
To the nurse
1)     "I want 2 wide pore gauge 14-16 IV lines established, please.
2)     Start Normal Saline/ Ringer Lactate, one liter on each at 125-1000 ml/h each (choose).
3)     Take a blood sample and send for:
- Blood group, Rh & cross-match,
- CBC, Lytes, (ABGs, CK-MB, Tropinin) if respiratory/cardiac case
- Liver function test (ALT, AST, ALP, & amylase),
- Renal function tests (BUN, Cr),
- Coagulation profile (INR/PTT),
- Rapid bedside Blood sugar,
- Toxicology screen (if indicated).
4)  Also send for:  12- lead ECG, CXR, Head CT (if comatose),
C-Spine & pelvic X-rays (if trauma)"
5)  Foley's catheter / Nasogastric tube (if needed).
"Mr/Ms .., I'll put a bee tube inside in order to monitor your urine output, okay..?"
Note: If blood seen from meatus: NO FOLEY'S (? Urethral injury)
 
Assess Respiratory Function:
Change to assisted ventilation or ETT if needed.
 If BP low:
 
 If Comatose:
 
3. Rule out shock:
 
 
 
 Â Ã‚ Ã‚  FFF  Go to Detailed Secondary Survey
 
 
FFF  Go to management below.
 
Management of hemorrhagic shock:
 
1 - Ask for vitals again: "What are his/her vitals, please?"
Carefully listen to what the examiner says and comment: e.g. "Normal/ so, he has fever/tachycardia/ tachypnea….".
2 - Secure airway and O2: Already done but check the mask and O2 level.
3 - Control bleeding by:
Find out them at the ebook " A Step By Step Guide To Mastering The OSCEs"
4 - Replace lost blood:
5 - Vasopressors:
-          Not during bleeding.
-          Used if hypotension persists despite appropriate volume administration. Also for septic and anaphylactic shocks.
 
* If Patient is stable now: Proceed to Disability.
If not: Repeat ABC until becomes stable.
 
Then you have to  go into D, E and then detailed survey.

We also asked you to prepare steps for every emergency management of common emergencies like:
** IF Diabetic Emergencies:
 
To the nurse "Run the Normal Saline at 1000cc/h each;
Give 5 (-10) IU Insulin IV bolus, then another 5 (-10)/h by IV infusion"
"When Blood glucose reaches 15 mMol/L change the fluid to two third 5% dextrose water (D5W) and one third Normal Saline. Then add 20 mEq/L KCL to the fluid."
"Send for urine glucose and ketones."

Dear Friends,
 
Kindly note that Department of Community Health Sciences (CHS) of Aga Khan University (AKU), Karachi, Pakistan  is pleased to announce its short course on Community-Based Social Development for Women’s Empowerment (CBSD-WE) scheduled from April 7 – 26, 2008. This course is part of CHS’ Continuing Education Programme.  Course information is given below for your reference. For course registration form please write to us.
 
Goal of the Course:
This course is designed to provide an understanding of the interplay between the determinants of health and development, self-awareness, and the importance of participatory methods in community mobilization and empowerment.
 
Course Objectives:
By the end of the course, the participants would be able to:
1.                       Understand the essential concepts and dynamics of social change.
2.                       Understand the essential features of participatory methodology, and its applications for facilitating social change.
3.                       Become aware of the relevance of self-development and importance of communication skills for community development.
4.                       Be able to critically engage with the different dimensions of empowerment in development
5.                       Become familiar with theory and research in the field of gender studies
 
Medium of Instruction:
Medium of instruction of the course is English. Participants are expected to be proficient enough to comprehend, and read and write in English language. However, flexibility is maintained in view of the purpose of the course, which is to advance critical understanding of development and its processes, and to acquire certain skills for facilitating people-centered and gender sensitive development. The learning environment created for this purpose allows the participants to converse in their own language. If needed and agreed by the participants, translations in other languages are also provided in the plenary sessions. For a collective learning process, such negotiations have been found to be useful, and in keeping with the participatory approach of the course, especially since one of the learning objectives is to introduce the participatory approach to the participants.
 
Teaching Method:
The entire course uses the workshop method, with minimal lecture sessions. Great emphasis is placed on experiential learning, and participants are required to monitor their own learning. Participatory tools for reflection and analysis, simulation-games and exercises will be used.
 
There is equal emphasis on individual and collective learning. Participants will be required to verbalize as well as write out their feelings and thoughts and to share them in small and large groups.
 
A significant feature of the course is a week of residential training in Participatory Reflection and Analysis (PRA) in an urban/rural area.
Tuition Fees:
The course tuition fees is Pak Rs. 24,200.00  (US$ 405.00) per participants, which covers classroom instructions, course material, refreshments, simple lunch during working hours, and boarding & lodging facility during one week of PRA workshop. 
 
Travel, boarding and lodging during the course period (except PRA component), and self-care arrangements will be the responsibility of the participant(s) or their parent organization(s).

JOHANNESBURG, 18 February 2008 (PlusNews) - The first microbicide candidate to reach the final phase of testing has failed to prevent HIV transmission, researchers announced on Monday. Testing of the microbicide, Carraguard, was carried out over a three-year period on 6,000 women in South Africa, and was completed in March 2007. But there was no difference in HIV infections between women in the group using Carraguard compared to the placebo group.

"The trial ... was unable to demonstrate Carraguard's efficacy in preventing HIV transmission," noted Dr Khatija Ahmed, principal investigator of the trial. The microbicide developed by the Population Council, an international non-profit organisation, contains carrageenan, which is derived from seaweed and widely used in the food and cosmetics industries as a gel, stabiliser and thickening agent. Laboratory, animal and early human tests suggested it might prevent HIV and other sexually spread infections, but Ahmed admitted that "what Carraguard showed in the lab couldn't be converted to humans".

She suggested that the low adherence rate could have been a factor: women who participated in the study used Carraguard less than half the number of times they had sex, and only 10 percent said they used it every time as directed. However, condom use shot up from 33 percent when the study began, to 64 percent. While acknowledging that the news was a disappointment, Ahmed stressed that the Carraguard trial was a "major milestone for microbicide development", as the trial had been completed with no safety concerns being raised.

This is another setback in the race to develop an effective microbicide - applied via a range of products like gels, films and sponges - that could help women prevent the transmission of HIV and other sexually transmitted infections. In 2000, a large full-scale trial showed that nonoxynol-9, a potential microbicide, was unsafe after women in the study developed a higher risk of HIV infection.

Seven years later, microbicide research was dealt another blow when the US-based reproductive health research organisation, CONRAD, announced the premature end of trials of a cellulose sulphate-based microbicide after the data safety and monitoring committee found a higher number of infections in the active group compared to the placebo group.

Advocates and researchers are reluctant to describe this trial as a setback. Fiona Scorgie, programmes coordinator at the Gender AIDS Forum, a non-governmental organisation monitoring microbicide trials in South Africa, told IRIN/PlusNews that although the end result had been disappointing, the trial had been "successful on another level".

The women participating in the trials had benefited from regular health screenings, while the safety of Carraguard meant that it could be used in future microbicide trials as a "vehicle for more specific substances, like antiretrovirals", but further development was needed. According to Scorgie, communities also had to be involved in the process, rather than being passive recipients. "Communities have a very important role to play ... it's important that we inform ourselves and remain critical".

Patients with advanced liver disease due to alcoholism have the same
survival rate after a liver transplant as nonalcoholic patients do, a
University of Pittsburgh study reports.

About 6 percent of the 666 adult patients who received a liver
transplant in Pittsburgh between 1980 and mid-1987-41 patients in all-
were alcoholics. Their one-year survival rate was 73 percent, which is
not significantly different from that of the 625 other liver
transplant patients, the study found.

An estimated 500,000 Americans traveled outside the United States for medical care in 2006, and a large increase was projected for 2007 as part of the rapidly growing phenomenon of medical travel.
In 2005, hospitals in Thailand treated 1 million patients from other countries. Hospitals elsewhere that reported treating high numbers of patients from other countries included those in Singapore (375,000 patients), the Philippines (250,000), India (150,000), and Malaysia (100,000).
Medical travelers from the United States leave for a single, compelling reason: low cost. Almost all of these patients are uninsured. The cost of major procedures abroad is usually one-fifth to one-eighth the cost of those performed in the United States, so any travel expenses are insignificant.
In the past, most U.S. medical travelers sought cosmetic surgery, which was rarely covered by insurance. Now, just 15% of patients travel for cosmetic surgery. Many patients combine their postoperative recovery with vacations arranged by a medical travel organization. Approximately 50 medical tourism companies in the United States offer services. These companies charge a processing fee of a few hundred dollars, and receive a commission from the hospital to which the patient is sent. In addition, medical tourism organizations recently formed an association.
Medical insurers and employers are discussing the possibility of outsourcing expensive procedures for their subscribers. Blue Cross/Blue Shield of South Carolina has formed a medical travel organization as an affiliate, and their subscribers are covered for medical and surgical care outside the United States.
The Joint Commission has given accreditation to organizations outside the United States since 1999 through its subsidiary, Joint Commission International (JCI). The mission of JCI is “to improve the safety and quality of care in the international community through the provision of education, publications, consultation, and international accreditation.”
Currently, 170 institutions in 22 countries are accredited by JCI. In addition to conducting educational activities, JCI sets standards that are comparable to those used by the Joint Commission in the United States and determines performance against the standards through triennial site visits by trained surveyors.
Unless the incentive for medical travel is removed by the introduction of universal health insurance in the United States, the phenomenon is likely to increase. The globalization of health care is underway, and the implications for the American health care industry are both obvious and disconcerting.

Starting 29 February 2008, the United Kingdom will be replacing the extension rules for the Highly Skilled Migrant Programme (HSMP) with the new Tier 1 system for General Highly Skilled Migrants. The new rules are part of its five-tier points based immigration system that will be implemented over the course of the next year.

After 29 February, any migrants currently within the United Kingdom seeking to extend their leave to remain as a highly skilled migrant will be required to score enough points under the Tier 1 points based scoring system for General Highly Skilled Migrants.

The number of points a migrant must score to extend their visa under Tier 1 remains the same as the current HSMP rules. However, the MBA provision will be removed and migrants can claim points for UK work experience. English language requirements are also more stringent, approximately equal to an overall band score of 6.5 on the International English Language Testing System (IELTS).

In April, people from India seeking to enter the UK as a highly skilled migrant for the first time will be required to come under the new Tier 1 rules. In the summer of 2008, the rest of the world will follow.

The Australian Medical Association (AMA) is calling for urgent action to increase the number of GPs in regional Victoria, setting its hopes on doctors considering immigration.

Shocking new figures show that the state has gained only one GP between 1996 and 2006. But Victoria's population has grown by more than 500,000 within that decade, meaning that it's in dire need for doctors. It's clear that more doctors must be brought in, either via other regions in Australia or immigration, because many GPs are suffering burn-out as figures show a huge jump in patient-doctor ratios. They're suffering health problems and chronic stress, and some have been forced to close their books to new patients.

And it's no wonder why. Health chiefs claim that half a million new Victorians are sharing just one GP. Federal Government figures revealed the average number of patients per GP rose from 759 in 1996-07 to 841 in 2006-07. In rural areas the shortage is even worse, where there can be more than 1700 patients for every doctor.

"Many Victorians, in regional and metropolitan areas, simply cannot get an appointment," said AMA state Vice-president Harry Hemley, who fears that the situation will only get worse as many older doctors leave the workforce.

Health chiefs are urging the federal and state government to increase the amount of doctors in order to resolve the massive shortage of general practitioners, meaning that immigration could bring in the numbers needed.  Areas that are feeling the doctor pinch the most are Ballarat, Bendigo, East Gippsland, Wodonga, Mornington Peninsula, Knox and the Goulburn Valley, while the AMA has predicted that North-East Victoria will feel it quite badly over the next five to 10 years if something is not done.

"In the next five years, Victoria will face the largest shortage of doctors the state has ever seen," said Dr Hemley. "The State and Federal Governments should ensure we get new GPs trained as soon as possible.''Even though there has been an increase in student doctors, it will be up to 10 years before they can actually start to practise.  "It'll take until about 2010, 2011 when the number of doctors graduating will double in Victoria, but still they've got to go through the training program and that takes another five years to be a GP," said Dr Hemley.

Currently, there are 130 vacancies for GPs in Victoria, and by 2012 an estimated 700 more doctors will be needed to keep up with demand in rural areas. There is now an increasing reliance on foreign doctors. In fact, about 25% of Victoria's doctors were originally trained overseas. But more need to be brought in via immigration if we want to fix the problem right now.

Dear All,

The Deptt.of Plastic  Surgery and Professional Development Center, Dow University of Health Sciences, Karachi, Pakistan, are holding a two day live workshop titled “Modern techniques of Hypospadias Surgery” on 31st March & 1st April, 2008, at Dowites 78 Operation Theatre Complex , Dow University of Health Sciences, from 9.00 am to 6.00 pm.

The target participants for this workshop are Consultants and post graduate students of General Surgery, Plastic Surgery, Pediatric Surgery and Urologic Surgery.

Course Instructor would be Dr.Obaidullah, Professor of Plastic Surgery, Peshawar Medical College. He is known internationally as a renowned Hypospadias Surgeon. He had worked with Bracka and Snodgrass. He had been invited as guest speaker at many international forums and had conducted Hypospadias workshop at Egypt, Turkey and India.

It will be a two day live surgery interactive program. Six participants would be given status of “Hands on Participants” on first come first basis. They will be directly involved in assisting and learning the techniques from Prof.Obaidullah.30 participants would be given status of “Observer Participants”. They will be watching the live surgery in the auditorium of Dowites 78 operation theatre complex. They will be free to communicate with Prof.Obiadullah through moderator of the session.

Registration is mandatory. Rs. 6000/= for “Hands on Participants” and Rs.500/=for “Observer Participants” will be charged for course material, DVDs, certificates and refreshments

Integrated health care: from research to policy and practice

Exciting Opportunities for Health, Social Science, Computer Science and Humanities Graduates

This structured PhD programme, funded by the Irish Health Research Board, commenced in 2007 and has an annual intake of postgraduate Scholars over 4 years. The programme is a joint initiative of senior academic staff from the Royal College of Surgeons in Ireland, Trinity College Dublin and University College Cork. They constitute the Health Services Research Institute. The Programme theme is "Integrated healthcare: from research to policy and practice".

This 4-year PhD Scholars Programme features:

  • 10-week specialist rotations in Irish health-related data collection agencies in the first year
  • Choice of PhD project from an extensive range during the first year
  • Opportunities for multidisciplinary research
